Caregivers Support Programs

The Caregiver Support Program provides services and resources to persons involved in an on going caregiving relationship. There is no cost to the caregiver or client for this program.

Potential Services:
* In-home respite care
* Adult Day Center, Assisted Living Facility or Nursing Home respite care.
* Care related educational opportunities for the caregiver (ie: classes, seminars, workshops, support groups...)
* Money towards equipment to enhance the caregiver's ability to provide care (i.e.: lift chairs, tub benches, hand held showers)

Eligibility:
Either the person receiving care or the person providing care must be 60 years of age or older and the client must reside in either Routt, Moffat, Garfield or Rio Blanco Counties.

If you or the person for whom you are caring are currently receiving services from Northwest Colorado Options for Long Term Care, please call you Case Manager before applying for their program. If not

Services under this program could include:
Homemaker (light housekeeping, cooking, assistance with bills and correspondance, Chore Service, Home maintenance, Home repair or renovation.

Equipment and Materials could include:
smoke detectors, eyeglasses, electronic monitoring, assistive and adaptive devices, hearing aids, dentures, prescriptions.

The Senior Equipment and Services Program was established through finding from the Area Agency on Aging (AAA) and the Older americans Act (OAA)to provide equipment, materials and services for older adults in an effort to help maximize their independence and safety.

Equipment available must be related to the safety and/or independence of the recipient in his/her home. The recipient must be 60 or older and live in Moffat, Routt, Rio Blanco or Garfield County. Preference will be given to those of need.
